Individual / Family Rates:
- Couples pricing 10% off base price
- Family rate Price Ceiling of $200 per month for a family of 4
2 adults and 2 children (ages 3-18) - each additional child $25

***Patients who terminate the agreement early, and wish to reenroll at a later date, you will be accepted on a "space available" basis subject to an enrollment fee.***
Business Rates:
- Employer groups with 5+ employees (staff auto opt in) $50/mos/adult.

Membership rates are for outpatient services. Dr. Trevino does not provide Emergency or Hospital care. If hospitalized you will be seen by a Hospitalist, when discharged you will follow up with Dr. Trevino for further management and care coordination from your hospital stay. (ER or inpatient)
Dr. Trevino's direct primary care memberships are NOT health insurance!
She recommends ALL patients to have health insurance to cover for hospital care, specialists, and advanced treatments and modalities.
